Individual $159 per month
Spouses/Partnerships $279 per month
Family* (Up to 5 people) $399 per month
*additional members on Family Plan $109 per month
We strongly encourage you to have insurance coverage for expenses such as prescription drug coverage, inpatient hospitalization, emergency care, surgeries, referral to specialists, physical therapy, occupational therapy, in home nursing, equipment or other services.
